Privacy Notice for our request forms

This notice explains what My Legacy Management collects when you use the request form at mylegacymanagement.com/start or the download form on our free resources pages (mylegacymanagement.com/resources and mylegacymanagement.com/es/recursos), why we collect it, and what you can ask us to do. It covers these forms only. Our Privacy Policy covers the rest of the website.

Who we are

My Legacy Management is a licensed insurance agency (CA Insurance License #0M17715) at 3186 Vista Way, Suite 312, Oceanside, CA 92056.

What we collect

What you type: your name, phone number and email address.
Your text-message choice: whether you ticked the optional text-message box, the exact wording that was on the page, and the date and time you sent the form.
How you reached the page: the campaign tags in the link you clicked (for example, which post or ad it came from), the website that sent you here, and this page’s address.
Which resource you asked for: on the resources pages, the name of the guide or download you requested.
A spam-control code: a short code made from your internet connection, used only to block spam and repeated sends. Many other connections share the same code, so it does not identify you or your device.

This form does not ask for your date of birth, Medicare number, Social Security number, health information or payment details. Please do not type them into the form. These pages do not set cookies and do not run advertising or analytics scripts of their own. On the resources pages, after you send the form your browser keeps a small note on your own device so the downloads stay unlocked; that note is not a cookie and is not sent to us. You can clear it in your browser settings.

Why we collect it

To contact you by phone or email about your request and the topics named on the form you used.
To send you text messages, only if you ticked the box.
To keep a record of what you agreed to.
To keep the form safe from spam and misuse.

Our customer system may automatically give a new request a priority score, which helps us decide whom to contact first. It does not decide anything about your coverage or eligibility. A licensed agent handles your request. We do not use this information to build advertising audiences.

Who receives it

We do not sell your personal information, and we do not share it for cross-context behavioral advertising. We do not share mobile numbers or text-message consent with third parties or affiliates for marketing or promotional purposes.

Some service providers handle it for us, only to run this form: our website and database host (Cloudflare), our own customer-record system, and a private team messaging app that alerts us when a new request arrives (the alert includes your name, phone number and email). We may also disclose information when the law requires it.

How long we keep it

We keep your request while we are working with you, and for as long as the law requires us to keep records of insurance contacts and consent. You can ask us to delete it (see below), and we will, unless the law requires us to keep it.

Your choices and California rights

You can ask us at any time to tell you what we hold about you, give you a copy, correct it, or delete it. Because we do not sell or share personal information, and we do not collect sensitive personal information through this form, there is nothing to opt out of or limit. We will not treat you differently for asking.

To ask, email [email protected] or call (760) 642-1892. We will check your request against the details you sent us and answer within 45 days. An authorized agent may ask on your behalf. To stop texts, reply STOP to any message. To stop calls or email, tell us and we will add you to our do-not-contact list.

Children

This form is for adults. We do not knowingly collect information from anyone under 18.

Changes

If we change this notice, we will update the date at the top of this page.
